Click Here to download the Medical Claims Form (PDF File) The state must be notified through this Medical Claims Form. The form must be completed and mailed to the state within 30 days of the injury. The MYSA Board must be notified as soon as possible if any player is injured during practice or a game and the injury requires medical attention (hospital, doctor, clinic, etc.). Please call or email the Travel Director.
